INTRODUÇÃO: As ressecções hepáticas minimamente invasivas vêm sendo realizadas com frequência crescente. O fígado é facilmente abordável através de laparoscopia para o tratamento de lesões localizadas nos segmentos anteriores e laterais; porém, o acesso aos segmentos posteriores é infrequente e tecnicamente difícil. OBJETIVO: Descrever a técnica empregada no acesso laparoscópico transtorácico para realização de ressecção hepática. TÉCNICA: Ressecção hepática em cunha no segmento 8 do fígado realizada através de laparoscopia com auxílio de portais transtorácicos. CONCLUSÃO: A abordagem transtorácica permite o acesso à parte posterior e aos segmentos superiores do fígado; deve ser considerada para tumores localizados excentricamente, e em pacientes com numerosas intervenções abdominais anteriores.

INTRODUCTION: Minimally invasive laparoscopic liver surgery is being performed with increased frequency. Lesions located on the anterior and lateral liver segments are easier to approach through laparoscopy. On the other hand, laparoscopic access to posterior and superior segments is less frequent and technically demanding. AIM: Technical description for laparoscopic transthoracic access employed on hepatic wedge resection. TECHNIQUE: Laparoscopic transthoracic hepatic wedge resection on segment 8. CONCLUSION: Transthoracic approach allows access to the posterior and superior segments of the liver, and should be considered for oddly located tumors and in patients with numerous previous abdominal interventions.

CONTEXTO: A hepatectomia é o tratamento de eleição para metástases hepáticas de câncer colorretal e diversos estudos têm demonstrado bons resultados, com índices de sobrevida em 5 anos entre 40% e 57%. Vários fatores clínico-patológicos preditivos de sobrevida após a ressecção hepática têm sido estudados e o envolvimento linfonodal do hilo hepático, que varia entre 2% e 10%, confere a este grupo prognóstico reservado a longo prazo. RESULTADOS: Embora com resultados variáveis, alguns autores têm relatado sobrevida não desprezível em pacientes com metástases hepáticas associada à doença linfonodal hilar submetidos a hepatectomia conjuntamente à linfadenectomia. Muitos centros especializados, embasados nos baixos índices de morbimortalidade da hepatectomia, realizam a ressecção hepática associada à linfadenectomia em casos selecionados. Cumpre ressaltar que o valor terapêutico da linfadenectomia sistemática ainda não é inteiramente conhecido e somente estudos controlados, comparando grupos com e sem linfadenectomia, poderão dirimir estas questões. CONCLUSÃO: De qualquer forma, a dissecção linfonodal hilar demonstrou ser uma ferramenta que torna mais preciso o estadiamento da doença extra-hepática, independente do impacto deste procedimento na sobrevida.

CONTEXT: Hepatectomy is the treatment of choice for colorectal liver metastases, and several studies have shown good results, with 5-year survival rates ranging from 40% to 57%. Several clinical and pathological predictive factors for survival after liver resection have been studied. Involvement of the hepatic hilum lymph nodes, the incidence of which varies from 2% to 10%, indicates a poor long-term prognosis. RESULTS: Despite variable results, some authors have reported a not-insignificant improvement in survival rate in liver-metastasis patients with hilar lymph node involvement who undergo combined liver resection and lymphadenectomy. Due to the low rates of morbidity and mortality for liver-resection surgery, several specialized centers perform liver resections combined with lymphadenectomies in selected cases. It should be noted that the therapeutic value of systemic lymphadenectomy is not yet entirely understood, and only controlled studies comparing groups with and without lymphadenectomy can fully resolve the issue. CONCLUSION: In any case, hilar lymph node dissection has been shown to be a useful tool for improving the accuracy of extra hepatic disease staging, regardless of its impact on survival.

OBJECTIVE: To test the hypothesis that short periods of ischemia may increase the myocardial protection obtained with intermittent crossclamping of the aorta. METHODS: In the control group (18 patients), surgery was performed with systemic hypothermia at 32ºC and intermittent crossclamping of the aorta. Extracorporeal circulation was used. In the preconditioning group (17 patients), 2 crossclampings of the aorta lasting 3min each were added prior to the intermittent crossclamping of the conventional technique with an interval of 2min of reperfusion between them. Blood samples for analyses of pH, pCO2, pO2, sodium, potassium, calcium, and magnesium were obtained from the coronary sinus at the beginning of extracorporeal circulation (time 1), at the end of the first anastomosis (time 2), and at the end of extracorporeal circulation (time 3). RESULTS: No difference was observed in the results of the 2 groups, except for a variation in the ionic values in the different times of blood withdrawal; sodium values, however, remained stable. All patients had a good clinical outcome. CONCLUSION: The results of intermittent crossclamping of the aorta with moderate hypothermia were not altered by the use of ischemic preconditioning.

Objetivo: Este estudo testa a hipótese de que curtos períodos de isquemia podem aumentar a proteção obtida pelo pinçamento intermitente da aorta. Métodos: No grupo controle (18), a operação foi realizada com hipotermia sistêmica a 32 ºC com pinçamento intermitente da aorta e uso de circulação extracorpórea (CEC). No segundo grupo, denominado de pré-condicionamento (17), foram acrescidos dois pinçamentos de 3 minutos da aorta com intervalo de 2 minutos de reperfusão entre eles, previamente ao pinçamento intermitente da forma convencional. CK-MB, troponina I, adenosina e lactato foram obtidos do seio ocoronário no início da circulação extracorpórea (1), ao final da segunda anastomose (2) e ao final da CEC (3). Resultados: Os níveis de CK-MB e troponina I apresentaram uma leve tendência a aumentar ao final da CEC no grupo controle, enquanto os de adenosina e lactato não apresentaram diferença. <img SRC="http:/img/fbpe/rbccv/v16n1/5182q1.gif"> Conclusão: Concluímos que o pré-condicionamento isquêmico não promoveu melhora significante na proteção miocárdica.

Background: This study tests the hypothesis that initial brief periods of ischemia could improve the protection obtained by intermittent aortic cross-clamping. Methods: In the control group (18), the procedure was performed under intermittent aortic cross-clamping at 32ºC. Patients in the preconditioned group (17) received a stimulus of two 3-min periods of cross-clamping followed by 2 min of reperfusion prior to standard operation. CKMB, troponin I, adenosine and lactate were obtained from the great cardiac vein at the onset of cardiopulmonary bypass (CPB) (1), at the end of the first anastomosis (2) and at the end of CPB (3). Results: CKMB and troponin I presented a slight trend to be higher at the end of CPB in the control group, while there was no difference between adenosine and lactate levels. <img SRC="http:/img/fbpe/rbccv/v16n1/5182q2.gif"> Conclusion: Ischemic preconditioning showed no significant improvement in myocardial protection.
